Position Description

A progressive, value-based healthcare organization in the area is seeking Nurse Practitioners, with a passion for serving the geriatric population, to join their team! This company specializes in caring for patients with complex, chronic conditions. They also have a great reputation for providing whole health to their patients.

As a Field Nurse Practitioner (NP), you will be responsible for improving patients’ quality of life from the comfort of their homes. The NP will work together with the interdisciplinary team to improve patients’ medical, social, and emotional health.

Nurse Practitioner (NP) Responsibilities Include:

  • Collaborate with the interdisciplinary team managing the health and wellbeing of patients living in their homes
  • Provide primary care to patients in the home and urgent care as needed
  • Deliver palliative care, and when needed, end of life care to patients and families
  • Perform initial patient screenings/physical examinations and create appropriate assessment/care plans
  • Order, interpret, and analyze diagnostic tests and determine the need for follow-up appointments and further treatment options
  • Prescribe and manage medications and treatments as appropriate, including but not limited to psychopharmacological agents

Schedule: Monday – Friday, 8:00am – 5:00pm

  • No nights, No weekends
  • Minimal On-Call

Salary: Based on years of Nurse Practitioner (NP) experience

  • 10% Bonus paid Quarterly

Qualifications:

  • Unrestricted, active NP license in the state of OR
  • NPI#
  • Board Certified
  • DEA License, or eligibility to obtain
  • 1+ years NP experience
  • BLS and ACLS certified
  • Valid driver’s license, car insurance, and reliable transportation
  • Flu Vaccination required
