Health Care Transition for youth and young adults with Autism

Please participate in an important survey of (MD, DO, ARNP, PA) primary care providers in Washington State, so we may better support your practice in caring for adolescents and young adults with Autism Spectrum Disorder as they transition to adulthood.
This survey is conducted by the UW LEND (University of Washington Leadership Education in Neurodevelopment Disorders), a program of the Maternal and Child Health Bureau located at the Univesity of Washington Center for Human Development and Disability in cooperation with Seattle Children's Hospital. The results of this survey will be used to create relevant and accessible educational and practical supports for primary care providers in Washington.
The questionnaire will take 5 minutes to complete and be confidential.
Once completed, you will be included in a raffle to win one of two $100 gift cards.
