Pediatric Oral Health Survey

You are invited to participate in an important survey regarding pediatric oral health practices in primary care (MD, DO, ARNP, PA).

Your participation in this project is voluntary. You can choose not to complete a question or to withdraw from the survey at any time. Given the public health issue of oral care for children, it is very important to learn your thoughts. Your opinion is vital as we take a closer look at this issue in our system. The survey will take less than 5 minutes to complete and answers will be kept strictly confidential. Results of this survey will be used to integrate oral health programs into primary care clinics.
