Social Determinants of Health: Participate in a Focus Group

The Washington Center for Nursing (WCN) is setting up focus groups now through mid-March to talk with nurses and other stakeholders involved in care coordination and transition management about the social determinants of health. WCN needs participants, as well as leaders who are able to help secure a date, time and location – for instance, embedding a focus group into an existing staff meeting. A flyer on this project is attached, along with a background document. Both can be edited to fit your needs, and WCN can provide other forms of communication for social media as well.
